Supervision failure highlighted at inquest into boy’s carbon monoxide death
An inquest in Queenstown heard that supervision failures by a gasfitter may have contributed to a 12-year-old's death from suspected carbon monoxide poisoning.
Jesse Samuel, aged 12, died from suspected carbon monoxide poisoning while showering. The inquest heard that a gasfitter's supervision failure was a factor in the death. The boy's uncle told the inquest that warnings on the internal water heating unit would have alerted him to the danger had he read them.
