A coroner has found Corrections failed to properly assess risks before an Invercargill inmate died in his cell.
Nukuroa Strange died in his cell after being moved out of a unit designated for at-risk prisoners. The coroner found Corrections missed a required risk assessment before the death. The coroner has also criticised ongoing safety issues at Invercargill Prison, describing systemic problems as unacceptable.
